Peen Dillies is a part of The Band, also known as the Winner Winner, What's 4 Dinner Boys. He plays the cowbell, while the rest of the band sings/whistles, plays guitar, or plays the bongos. They are accompanied by the dancers Candy Simmons, Cherry Fleet, Destiny Woods, and Sapphire Dawn.
If Buddy takes her mask off, the entire band will jump her. If not, the band will continue playing peacefully.

- In the LISA: THE PAINFUL art collection, there is a Joy Mutant version of Peen Dillies and the violently killed bodies of the rest of the band.
- Although never appearing in Painful, these sprites could have indicated that Peen had mutated and killed his fellow band mates.
- He still plays his cowbell in mutant form.
